Rockford Urological Associates accepts most insurance plans. Prior to making your appointment, check with your insurance carrier to verify we are a network provider. If your insurance requires a referral, please contact your Primary Physician. It is your responsibility to obtain your referral prior to your appointment. Without a referral, you may be rescheduled.
A friendly reminder: patients are responsible for their co pays at the time of service. Updated insurance information is required at check in. We will file a claim with your insurance company, but any coinsurance and/or deductible will be billed to you. It is ultimately your responsibility to understand your individual plan. If you cannot keep your scheduled appointment please call our office to reschedule or cancel, all no shows will be billed a $40.00 no show fee.
